Location:    Bridges of Poplar Creek Country Club – 1400 Poplar Creek Dr., Hoffman Estates, IL 60195

There will be a luncheon following the tournament once play is complete. We are accepting golf reservation on a first-come, first-serve basis.  Golf is limited to the first 144 paid golfers.  Payment must be received by July 9^th to guarantee your tee time.

Golf course dress code:  No cutoffs, gym trunks, running shorts, t-shirts (men’s shirt must have a collar), no jeans of any kind, halter-tops or bathing suits.  These rules will be strictly enforced.

CCBFA Member Price

• Golf & Lunch* – $125.00
• Lunch Only – $60.00

CCBFA Non-Member Price

• Golf & Lunch* – $160.00
• Lunch Only – $75.00

If not paying on-line, please mail your payment for receipt by July 7th to:  CCBFA – P.O. Box 66365, Chicago, IL 60666. Payments must be received by July 7th to gurantee registration.

Terms and Conditions

• No registrations will be accepted after July 7^th
• No cancellations will be accepted after July 7^th; you will be billed otherwise
• Lunch includes a vegetarian option.  Please note on comment section if desired
• $5.00 of each ticket will go toward the CCBFA Scholarship Fund
